Take the next step in your career as a Home Health Physical Therapist. This contract role involves delivering compassionate physical therapy services to patients in their homes, empowering them to achieve their highest level of independence and functional ability. Youll perform comprehensive evaluations, create individualized care plans, and closely collaborate with interdisciplinary healthcare professionals to deliver high-quality, patient-focused care.

Schedule:

• Full-time, 13-week contract term
• Monday Friday, 8:00 AM 5:00 PM
• Field-based, daily travel to patient homes (mileage reimbursement provided)
• Caseload: Approximately 2532 points per week (57 patient visits/day)
• Electronic documentation with Home Care Home Base (HCHB) and OASIS assessments

Key Responsibilities:

• Complete thorough physical therapy evaluations, including required OASIS assessments for start of care, recertification, and discharge
• Develop and implement evidence-based treatment plans focused on patient mobility, strength, pain management, and safety
• Deliver skilled interventions, such as therapeutic exercise, gait and transfer training, fall prevention, and patient/caregiver education
• Monitor patient progress and modify care plans accordingly, maintaining proactive communication with physicians and care team members
• Maintain accurate, timely electronic documentation according to regulatory and agency standards
• Participate in interdisciplinary meetings and case conferences to ensure seamless patient care

Qualifications:

• Active Physical Therapist license (PT) in good standing
• Minimum 1 year of recent home health physical therapy experience required
• OASIS experience required; HCHB proficiency preferred
• Strong clinical, communication, time management, and organizational skills
• Reliable transportation, valid drivers license, and current auto insurance

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
