Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist (PT) license in Indiana, in good standing
- At least one year of recent home health physical therapy experience (field-based strongly preferred)
- OASIS assessment experience is required
- Proficiency in Home Care Home Base (HCHB) EMR preferred
- Excellent clinical judgment and patient assessment abilities
- Reliable transportation, valid drivers license, and auto insurance
- Exceptional communication, organizational, and time management skills
- Comfortable working independently in a field-based setting
Primary Responsibilities:
- Conduct comprehensive PT evaluations, including OASIS assessments for start of care, resumption, recertification, and discharge
- Design and implement individualized PT care plans targeting improved mobility, strength, safety, and functional outcomes
- Deliver hands-on interventions such as therapeutic exercise, gait and transfer training, balance and fall prevention, and pain management
- Educate patients and caregivers on adapted programs, exercise plans, safe practices, and assistive devices
- Document care accurately and efficiently in Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams to support coordinated, effective care
- Participate actively in case conferences and team discussions
