In this rewarding role, youll deliver skilled therapy services directly in patients homes, supporting their goals of improved mobility, independence, and quality of life. Your expertise will be crucial in evaluating needs, crafting targeted treatment plans, and partnering with a dedicated interdisciplinary team.

Schedule:

• Full-time contract, 13 weeks
• Monday Friday, 8:00 AM 5:00 PM

What You'll Do:

• Conduct comprehensive physical therapy assessments, including OASIS evaluations as applicable.
• Develop and implement individualized care plans focused on mobility, balance, strength, and safety.
• Deliver therapeutic interventions such as exercise programs, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
• Educate patients and caregivers about exercises, use of assistive devices, and safety in the home.
• Monitor progress, adapt plans as necessary, and communicate changes to physicians and care teams.
• Utilize Home Care Home Base (HCHB) for timely and accurate electronic documentation.
• Collaborate with nurses, occupational and speech therapists, medical social workers, and aides to ensure coordinated, comprehensive care.
• Participate in team meetings, case conferences, and ongoing education.

Qualifications:

• Active Physical Therapist (PT) license in good standing.
• At least 1 year of recent field-based home health PT experience.
• Proficiency with HCHB preferred; OASIS experience required.
• Reliable transportation and a valid drivers license.
• Exceptional clinical, assessment, communication, and organizational skills.
• Ability to work independently and adapt to a dynamic field-based schedule.

$1,718 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 32 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
