An experienced Home Health Physical Therapist (PT) is needed for a full-time, 13-week contract in Nicholasville, KY. This role involves providing skilled physical therapy services directly in patients homes, focusing on restoring mobility, strength, balance, and functional independence. Candidates should be self-motivated, organized, and comfortable managing a flexible, field-based schedule to deliver high-quality, patient-centered care.

Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments for start of care, resumption, recertification, and discharge.
• Develop and execute individualized treatment plans addressing functional mobility, pain management, gait, strength, balance, and safety.
• Provide interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Monitor patient progress, update treatment plans as needed, and communicate changes to physicians and interdisciplinary team members.
• Educate patients and caregivers on exercises, safety precautions, assistive device use, and home exercise programs.
• Document accurately and timely using the Home Care Home Base (HCHB) system.
• Collaborate with nurses, occupational and speech therapists, social workers, and home health aides to promote coordinated care.
• Adhere to all agency policies, Medicare guidelines, and regulatory standards.

Qualifications:

• Active Physical Therapist license in good standing for the state of practice.
• Minimum one year of recent home health physical therapy experience, with field experience preferred.
• Proficiency with Home Care Home Base (HCHB) electronic documentation system preferred.
• Required experience with OASIS assessment tools.
• Strong clinical decision-making and functional assessment abilities.
• Valid drivers license, reliable transportation, and auto insurance.
• Excellent communication, time management, and organizational skills.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Approximately 57 patient visits daily (2532 points per week)
