Job Description

Provide specialized physical therapy care in patients homes as a traveling clinician based in Newport, VT. This role focuses on outpatient home health and hospice services, requiring a skilled professional comfortable navigating a dynamic schedule and delivering high-quality therapy in various patient environments.

Key Responsibilities:

  • Conduct thorough evaluations and treatments using OASIS and other home health and hospice protocols.
  • Travel between patient homes efficiently while adapting to schedule changes and urgent care needs.
  • Utilize the HealthWyse EMR system for accurate documentation and communication.
  • Collaborate with interdisciplinary teams to develop and adjust individualized care plans.
  • Maintain professional standards and patient confidentiality during all visits.
  • Provide holiday coverage as required, working Monday through Friday, 8 a.m. to 5 p.m.

Qualifications:

  • Licensed Physical Therapist with active credentials.
  • Proven experience in home health and hospice physical therapy.
  • Strong familiarity with OASIS assessments and outpatient care procedures.
  • Ability to travel independently between patients residences.
  • Proficiency in using electronic medical records, specifically HealthWyse, is preferred.
  • Excellent organizational skills and adaptability to changing schedules.
