Job Description

An opportunity is available for an experienced and compassionate Home Health Physical Therapist (PT) to provide essential rehabilitative care to patients in their own homes. This is a field-based, full-time contract position focused on supporting individuals in regaining mobility, improving strength and balance, and maintaining functional independence through patient-centered care.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM

Primary Duties:

  • Conduct comprehensive physical therapy evaluations and OASIS assessments as required
  • Create and implement individualized treatment plans targeting mobility, balance, and pain management
  • Deliver skilled interventions, including therapeutic exercise, gait training, transfer training, neuromuscular re-education, and fall prevention
  • Monitor patient progress, modify plans as needed, and communicate status changes to physicians and care teams
  • Educate patients and caregivers on exercises, safety strategies, use of assistive devices, and home exercise programs
  • Accurately document care using Home Care Home Base (HCHB)
  • Collaborate with nurses, occupational therapists, speech-language pathologists, home health aides, and social workers for coordinated patient care
  • Participate in case conferences, interdisciplinary meetings, and professional development activities
  • Ensure compliance with agency policies, regulatory requirements, and Medicare guidelines

Required Qualifications:

  • Active, unrestricted Physical Therapist (PT) license
  • At least 1 year of recent home health physical therapy experience
  • Experience with OASIS and familiarity with Home Care Home Base (HCHB) preferred
  • Strong clinical judgment and functional assessment abilities
  • Reliable transportation, valid drivers license, and current auto insurance
  • Exceptional organizational, time management, and interpersonal communication skills
  • Ability to independently manage a flexible caseload in a community setting
