Step into a vital role as a Home Health Physical Therapist, delivering skilled physical therapy services directly to patients in their homes. This position is ideal for a dedicated, compassionate professional committed to supporting patients mobility, independence, and overall well-being.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

In this 13-week, full-time contract, your responsibilities will include comprehensive evaluations, development and implementation of individualized plans of care, and ongoing collaboration with a multidisciplinary healthcare team. Youll provide expert interventions such as therapeutic exercise, gait training, transfer and fall prevention strategies, and patient and caregiver education. Utilizing OASIS assessments where required, youll ensure accurate, up-to-date documentation through Home Care Home Base (HCHB). Daily travel to patient homes within an assigned territory is required, with mileage reimbursement provided.

Key Qualifications:

• Active Physical Therapist (PT) license in the state of practice and in good standing
• Minimum of 1 year recent Home Health Physical Therapy experience
• Familiarity with OASIS assessments required
• Proficiency with Home Care Home Base (HCHB) preferred
• Reliable transportation, valid drivers license, and auto insurance
• Excellent clinical assessment, communication, and time management skills
• Ability to work independently and manage a flexible, field-based schedule
