An opportunity is available for an experienced and compassionate Home Health Physical Therapist to provide essential care to patients in their homes. This position focuses on helping individuals achieve greater mobility, independence, and wellness by delivering skilled physical therapy services in a field-based setting. The ideal candidate values autonomy, excels in organization, and is adept at working both independently and collaboratively within an interdisciplinary healthcare team.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Primary Responsibilities:
- Perform comprehensive physical therapy assessments, including OASIS documentation as required.
- Develop and implement personalized therapy plans aimed at improving functional mobility, strength, and balance.
- Deliver skilled interventions, which may include therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
- Educate patients and caregivers on safe mobility, exercise regimens, and the effective use of assistive devices.
- Monitor patient progress and adapt treatment plans accordingly, ensuring clear communication with physicians and the wider care team.
- Accurately document all care activities using Home Care Home Base (HCHB).
- Participate in interdisciplinary meetings and contribute to care coordination.
- Adhere to established agency protocols, Medicare guidelines, and all relevant regulations.
Qualifications:
- Current Physical Therapist (PT) license in good standing for the state of practice.
- At least 1 year of recent experience in home health physical therapy.
- OASIS assessment experience is required.
- Proficiency with Home Care Home Base (HCHB) is preferred.
- Demonstrates strong clinical judgment, excellent organizational skills, and clear communication abilities.
- Reliable transportation, valid drivers license, and current auto insurance.
