Job Description
Provide skilled physical therapy services as a Home Health Physical Therapist (PT) delivering patient-centered care in home settings. This full-time, 13-week contract role involves supporting patients mobility, strength, balance, and functional independence through comprehensive evaluations and tailored treatment plans.
Key Responsibilities:
- Conduct thorough physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, Discharge)
- Develop and implement individualized plans of care focusing on mobility, pain management, gait training, and fall prevention
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, and transfer training
- Monitor patient progress and adjust treatments as needed, communicating changes with physicians and healthcare team members
- Educate patients and caregivers on exercises, safety, and assistive device use
- Maintain accurate documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members to ensure coordinated care
- Adhere to agency policies, Medicare guidelines, and regulatory standards
Qualifications:
- Active Physical Therapist (PT) license in good standing within state of practice
- Minimum 1 year recent Home Health PT experience, preferably in the field
- Proficiency with Home Care Home Base (HCHB) documentation preferred
- Experience with OASIS assessments required
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical decision-making, communication, time management, and organizational skills
- Ability to work independently with a flexible, field-based schedule
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload approximately 2532 points weekly (57 visits daily)
- Daily travel to patient homes within assigned territory (mileage reimbursed)
