An experienced Home Health Physical Therapist (PT) is needed to provide skilled physical therapy services directly in patients homes in the New Haven, CT area. This full-time, 13-week contract position offers an opportunity to deliver patient-centered care that enhances mobility, strength, balance, and functional independence among diverse home health patients. The role requires self-motivation, strong clinical skills, and the ability to work independently in a field-based setting.

Key Responsibilities:

• Conduct comprehensive evaluations including OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
• Develop and implement individualized PT plans focusing on therapeutic exercise, neuromuscular re-education, gait and transfer training, pain management, and fall prevention.
• Monitor patient progress, adjust treatment plans accordingly, and communicate updates to physicians and the interdisciplinary care team.
• Educate patients and caregivers on exercises, safety practices, assistive devices, and home exercise programs.
• Maintain accurate documentation within the Home Care Home Base (HCHB) electronic system.
• Collaborate effectively with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides.
• Participate in case conferences and interdisciplinary meetings while adhering to Medicare guidelines and agency policies.
• Travel daily to patient homes within the assigned territory; mileage reimbursement provided.

Qualifications and Experience:

• Active, good-standing Physical Therapist license for the state of practice.
• Minimum one year of recent Home Health Physical Therapy experience, with field experience preferred.
• Proficiency with HCHB is preferred; OASIS experience is required.
• Strong critical thinking, clinical decision-making, and time management skills.
• Reliable transportation with a valid drivers license and auto insurance.
• Excellent communication skills and ability to function independently in a flexible, field-based schedule.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• Caseload averages 2532 points weekly (approximately 57 patient visits daily).
