Job Description
Provide expert physical therapy services as a Home Health Physical Therapist (PT), delivering personalized care to patients in their homes to enhance their mobility, strength, balance, and overall independence. This contract position requires a motivated and organized professional comfortable managing a field-based caseload while collaborating with an interdisciplinary care team to achieve optimal patient outcomes.

Responsibilities:
- Conduct thorough physical therapy evaluations, including OASIS assessments
- Develop and implement individualized treatment plans addressing mobility, strength, balance, gait, pain management, and safety
- Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor patient progress, adjust care plans, and communicate changes to physicians and care team members
- Educate patients and caregivers on exercises, safety, assistive device use, and home programs
- Maintain accurate, timely documentation using Home Care Home Base (HCHB)
- Collaborate with nurses, occupational therapists, social workers, and home health aides for coordinated care
- Participate in case conferences, interdisciplinary meetings, and continuing education
- Comply with agency policies and Medicare home health regulations
Qualifications:
- Active Physical Therapist license in Connecticut, in good standing
- Minimum 1 year recent home health physical therapy experience; field experience preferred
- Proficiency with HCHB and required OASIS experience
- Strong clinical decision-making and functional assessment skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with excellent communication and organizational skills
Contract Details:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload of approximately 2532 points weekly (57 patient visits per day)
- Daily travel within assigned territory; mileage reimbursement provided
