A dedicated Physical Therapist with expertise in home health is needed to support patients in New Britain, CT, through a full-time, 13-week contract role. This opportunity focuses on delivering compassionate, patient-centered physical therapy services within patients homes, promoting recovery and functional independence.

Responsibilities include conducting thorough initial assessments of patients physical conditions and mobility, developing customized treatment plans aligned with patient goals and physician instructions, and administering therapeutic exercises, manual therapy, and other modalities. The role also involves educating patients and their families on exercise techniques and the safe use of assistive devices, monitoring progress, and collaborating closely with physicians, nurses, and occupational therapists to ensure coordinated and effective care. Maintaining precise electronic medical records and adhering to all regulatory and ethical standards is essential.

Qualifications and Desired Experience:

  • Bachelor's or Masters degree in Physical Therapy from an accredited institution
  • Current state license or eligibility for licensure as a Physical Therapist
  • Experience in a home health setting is required
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records software
  • Strong clinical reasoning and ability to adapt treatment plans based on patient progress
  • Excellent communication and interpersonal skills for effective collaboration
  • Understanding of Medicare regulations and home health documentation
  • Valid driver's license and reliable transportation
