Job Description

Physical therapists with a passion for home health are invited to make a meaningful difference by delivering individualized, compassionate patient care. In this role, youll visit patients in their homes, helping them regain and maintain independence through evidence-based therapy interventions. Your expertise will empower patients and families, while your clinical skills will support optimal recovery and quality of life.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Daily travel required within an assigned territory (mileage reimbursement provided)
  • Electronic documentation via Home Care Home Base (HCHB)
  • Caseload: approximately 2532 points per week, typically 57 patient visits a day

Responsibilities:

  • Complete thorough physical therapy evaluations, including OASIS assessments for appropriate visits
  • Develop, implement, and regularly adjust patient-specific plans of care targeting mobility, strength, balance, gait, and pain management
  • Provide therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention education in the home setting
  • Educate patients and caregivers on exercises, safety precautions, and appropriate use of adaptive equipment
  • Document care accurately and promptly in HCHB
  • Communicate changes in a patients status and progress to physicians and other healthcare team members
  • Collaborate with nurses, occupational and speech therapists, social workers, and aides for coordinated, holistic care
  • Participate in interdisciplinary meetings, case conferences, and ongoing education

Qualifications:

  • Active Physical Therapist (PT) license in good standing
  • Minimum of 1 year of recent home health PT experience required
  • Experience with OASIS assessments required
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Strong decision-making, assessment, organization, and communication skills
  • Reliable transportation, valid drivers license, and auto insurance
