An experienced and compassionate physical therapist is needed to provide high-quality care to patients in their homes. This contract position is ideal for a self-motivated clinician who thrives working independently while making a meaningful difference in patients functional abilities and quality of life.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough physical therapy evaluations, including comprehensive OASIS assessments as required
• Develop and implement evidence-based, individualized care plans focused on improving mobility, strength, balance, and safety
• Deliver skilled interventions such as therapeutic exercise, gait training, neuromuscular re-education, and fall prevention as appropriate
• Monitor patient progress, adjust plans of care as needed, and communicate effectively with the interdisciplinary team and physicians
• Educate patients and caregivers on home exercise programs, assistive device use, and safety strategies to ensure successful recovery
• Maintain timely, accurate electronic documentation in Home Care Home Base (HCHB)
• Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to ensure coordinated patient care
• Adhere to agency protocols, Medicare guidelines, and regulatory standards

Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice
• At least one year of recent home health physical therapy experience; field experience is highly preferred
• Required experience with OASIS assessments; proficiency in HCHB documentation is a plus
• Strong clinical decision-making and assessment skills
• Reliable transportation, valid drivers license, and auto insurance
• Outstanding communication, organization, and time management skills
• Ability to work independently and manage a flexible, field-based caseload
