Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel within assigned region (mileage reimbursement included)
• Caseload averages 2532 points per week (about 57 patient visits per day)

Key Responsibilities:

• Perform comprehensive physical therapy assessments, including required OASIS evaluations.
• Create and implement personalized care plans centered on mobility, strength, gait, fall prevention, and pain management.
• Apply skilled interventions, including therapeutic exercise, neuromuscular re-education, and transfer/gait training.
• Educate and support patients and caregivers on home exercises, equipment use, and safe practices.
• Monitor progress, update plans as needed, and communicate status changes to the care team and physicians.
• Accurately document all care activities in Home Care Home Base (HCHB).
• Collaborate with nurses, OT, SLP, social workers, and aides to ensure coordinated patient care.
• Participate in interdisciplinary meetings and ongoing professional education.

Preferred Qualifications:

• Current Physical Therapist license in good standing for the state of practice.
• At least 1 year of recent home health PT experience.
• Proficiency with HCHB documentation and OASIS assessments.
• Strong communication and clinical decision-making skills.
• Reliable transportation, valid drivers license, and insurance.
• Ability to organize, prioritize, and manage time independently.
