Bring your physical therapy expertise to a dynamic home health environment where your skills will support patients' independence and quality of life. As a Home Health Physical Therapist, you'll provide essential care directly in patients' homes, utilizing your clinical knowledge and strong communication skills to drive positive outcomes. This full-time, 13-week contract role requires a proactive and compassionate clinician ready to be an integral part of a collaborative interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive physical therapy assessments, including OASIS assessments as needed
- Create and execute individualized plans of care targeting mobility, strength, balance, and safety
- Offer interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor patient progress, adjust care plans as appropriate, and communicate effectively with both healthcare providers and families
- Educate patients and caregivers on safe practices, the use of assistive devices, and home exercise routines
- Complete timely and accurate documentation in Home Care Home Base (HCHB)
- Foster coordination with RNs, OTs, SLPs, social workers, and aides to optimize patient care
- Participate in case conferences and maintain compliance with agency policies and Medicare guidelines
Qualifications:
- Active Physical Therapist license in the state (in good standing)
- At least 1 year of recent home health PT experience required
- Proficient with Home Care Home Base (HCHB) and OASIS required
- Effective clinical and communication skills
- Self-directed, reliable, and organized with the ability to manage a field-based role
- Valid drivers license, reliable transportation, and auto insurance
