An experienced Home Health Physical Therapist (PT) is needed to provide skilled, compassionate care to patients in their homes. This role focuses on improving patients mobility, strength, balance, and overall functional independence through individualized treatment plans and collaborative care. Candidates should be self-motivated, organized, and adept at working independently while maintaining high standards of patient-centered therapy.

This position is based in Myrtle Creek, OR, a charming community known for its scenic beauty and outdoor recreational opportunities. Located near the Umpqua River, Myrtle Creek offers a peaceful environment with access to hiking, fishing, and community-oriented livingall contributing to a balanced lifestyle outside of work.
Responsibilities:
- Conduct comprehensive physical therapy evaluations, including required OASIS assessments
- Develop and implement individualized care plans targeting functional mobility, pain management, gait training, and fall prevention
- Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, and transfer training
- Monitor patient progress and adjust treatment plans accordingly, communicating changes to physicians and care teams
- Educate patients and caregivers on exercises, safety measures, assistive devices, and home exercise programs
- Maintain timely, accurate documentation in Home Care Home Base (HCHB) electronic system
- Collaborate with interdisciplinary team members including nurses, occupational therapists, and medical social workers
- Participate in case conferences and self-development activities
- Ensure compliance with agency policies, Medicare guidelines, and regulatory standards
Qualifications / Desired Experience:
- Active Physical Therapist license in Oregon (in good standing)
- Minimum of 1 year recent experience in Home Health Physical Therapy; field experience preferred
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Strong clinical decision-making and functional assessment skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently in a flexible, field-based schedule
- Excellent communication, time management, and organizational abilities
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Approximately 57 patient visits per day, with 2532 therapy points weekly
Contract Type:
- Full-time, 13-week contract with potential for extension
