A rewarding opportunity is available for a skilled and compassionate physical therapist with specialized experience in home health care. In this contract position, you will deliver quality physical therapy services in patient homes, supporting greater mobility and independence for those recovering from illness, injury, or surgery. This field-based role requires a proactive, organized PT dedicated to individualized patient care and successful outcomes.

Schedule & Structure:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: Approximately 2532 points weekly (57 patient visits per day, depending on visit types)
• Daily travel to patient homes within an assigned territory

Qualifications:

• Active Physical Therapist license (PT) in good standing for the state of practice
• At least one year of recent Home Health PT experience (field experience strongly preferred)
• Hands-on proficiency with OASIS assessments (required)
• Knowledge of Home Care Home Base (HCHB) electronic documentation (preferred)
• Reliable transportation with a valid drivers license and auto insurance
• Strong assessment, clinical reasoning, and communication skills
• Ability to independently manage a flexible, mobile schedule

Core Responsibilities:

• Conduct comprehensive home-based evaluations, including OASIS assessment as needed
• Develop, execute, and revise individualized PT plans to enhance mobility and safety
• Deliver evidence-based interventions: therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
• Track progress, modify plans when indicated, and communicate updates to the interdisciplinary healthcare team
• Educate patients and caregivers on exercises, adaptive equipment, and home safety strategies
• Ensure timely, accurate documentation via Home Care Home Base (HCHB)
• Actively participate in case discussions and continuing education
• Maintain compliance with agency policies, regulatory standards, and home health guidelines

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
