Job Description

Deliver meaningful care by working directly with patients in their homes as a physical therapist. This contract opportunity is ideal for a skilled and compassionate healthcare professional eager to support patients independence and mobility in a home health setting.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Perform comprehensive physical therapy evaluations and OASIS assessments as needed.
• Design and implement individualized care plans to address mobility, strength, balance, gait, pain, and safety goals.
• Deliver skilled therapy interventions, including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Regularly assess patient progress, adjusting treatment plans in coordination with the care team and physician.
• Provide patient and caregiver education on exercises, safety, assistive devices, and home programs.
• Maintain accurate, timely records using Home Care Home Base (HCHB).
• Engage in collaborative care by working with nurses, occupational and speech therapists, social workers, and home health aides.
• Attend case conferences, interdisciplinary meetings, and participate in ongoing education as needed.
• Ensure compliance with Medicare, agency policies, and applicable regulations.

Qualifications:

• Active Physical Therapist (PT) license (in good standing for the state of practice)
• Minimum of 1 year of recent home health PT experience (field experience strongly preferred)
• Familiarity with Home Care Home Base (HCHB) preferred
• Proficiency in OASIS assessments required
• Strong assessment, communication, and time management abilities
• Reliable transportation, valid drivers license, and auto insurance
• Self-motivated and able to work independently
