Job Description
Deliver compassionate, skilled physical therapy to patients in their homes as part of a dedicated healthcare team. This full-time, 13-week contract offers the opportunity to work independently while supporting patient mobility, safety, and independence in a field-based setting. You will play a key role in assessment, care planning, and educating patients and caregivers while collaborating with an interdisciplinary group of healthcare professionals.
Schedule & Assignment:
- Full-time contract, 13 weeks
- Monday to Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points weekly (roughly 57 patient visits per day)
- Daily travel to patient homes within assigned area (mileage reimbursement provided)
- Electronic documentation with Home Care Home Base (HCHB)
- Use of OASIS for assessment and compliance
Requirements:
- Active Physical Therapist license (in good standing) in the state of practice
- At least 1 year of recent home health physical therapy experience
- Experience with Home Care Home Base (HCHB) preferred
- Proficiency with OASIS assessments required
- Excellent functional assessment and clinical decision-making ability
- Strong organizational, time management, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Comfort working independently in a fast-paced, field-based setting
Key Responsibilities:
- Complete comprehensive evaluations and OASIS assessments
- Develop and implement tailored treatment plans
- Deliver interventions such as therapeutic exercise, gait and transfer training
- Monitor patient progress, update plans, and report status changes to the care team
- Educate patients and caregivers to promote safety and successful outcomes
- Maintain accurate, timely documentation
- Participate in interdisciplinary meetings and case conferences
