Job Description
A skilled and compassionate professional is needed to deliver high-quality physical therapy services in a home health setting. Working directly with patients in their homes, you will play a central role in developing and implementing care plans that promote mobility, functional independence, and improved quality of life.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
In this role, you will:
- Conduct comprehensive physical therapy evaluations, utilizing OASIS assessments where required.
- Develop and execute individualized treatment plans centered on mobility, strength, balance, gait, and safety.
- Deliver skilled interventions, including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
- Re-assess patient progress, update treatment plans appropriately, and coordinate with physicians and care team members.
- Educate patients and caregivers regarding safety strategies, assistive devices, and home exercise routines.
- Maintain timely, accurate documentation through Home Care Home Base (HCHB).
- Collaborate closely with RNs, OTs, SLPs, social workers, and home health aides to ensure coordinated, patient-centered care.
- Participate in interdisciplinary meetings and engage in continuous professional development.
- Comply with agency policies and all Medicare home health standards.
Requirements:
- Active, unrestricted Physical Therapist (PT) license in the state of practice.
- At least 1 year of recent home health physical therapy experience (field experience preferred).
- OASIS assessment experience required.
- Proficiency with HCHB a plus.
- Strong clinical skills, with the ability to work independently and manage a flexible schedule.
- Reliable transportation, valid drivers license, and current auto insurance.
- Excellent communication, organizational, and time management abilities.
