Deliver high-quality, patient-focused care as a Home Health Physical Therapist, providing essential therapy services directly in patients homes. Through your clinical expertise and compassion, you'll help individuals regain strength, mobility, and independence, making a positive difference every day.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based role with daily patient visits (expect about 57 visits per day)
• Daily travel within the assigned territory (mileage reimbursement included)

Responsibilities:

• Complete thorough physical therapy evaluations, including required OASIS assessments.
• Develop, implement, and adjust individualized care plans centering on mobility, balance, gait, pain management, and safety.
• Deliver skilled interventions, such as therapeutic exercises, neuromuscular re-education, and fall prevention techniques.
• Educate patients and caregivers on exercises, safety, assistive devices, and ongoing home exercise programs.
• Track patient progress, update physicians and coordinate with an interdisciplinary team (including nurses, OTs, SLPs, and social workers) to ensure optimal outcomes.
• Maintain timely, accurate documentation in Home Care Home Base (HCHB).
• Attend required case conferences, interdisciplinary meetings, and maintain ongoing professional education.
• Follow all agency, regulatory, and Medicare guidelines to deliver compliant, high-standard care.

Qualifications:

• Active Physical Therapist license (in good standing) for the state of practice.
• Minimum of one years recent home health PT experience.
• OASIS assessment proficiency required.
• Experience with Home Care Home Base (HCHB) strongly preferred.
• Strong clinical assessment, time management, and communication skills.
• Reliable vehicle, valid drivers license, and insurance.

$2,247 - $2,377/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
