Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload of approximately 2532 points per week, typically 57 daily patient visits
- Travel within an assigned local territory; mileage reimbursement provided
Key Qualifications:
- Valid and active Physical Therapist license in the state
- At least 1 year of recent home health physical therapy experience (field experience valued)
- Experience with OASIS essential; Home Care Home Base (HCHB) proficiency preferred
- Ability to perform clinical assessments and create individualized care plans
- Reliable transportation, current drivers license, and auto insurance
- Strong organizational, communication, and time management skills
- Comfortable working independently and collaborating with an interdisciplinary team
Primary Responsibilities:
- Complete thorough physical therapy evaluations, including OASIS assessments as required
- Develop and deliver customized treatment plans aimed at improving functional mobility, balance, and strength
- Provide skilled therapeutic interventions and patient/caregiver education on safety and home exercise programs
- Monitor and document patient progress in HCHB, adjusting plans as appropriate
- Work closely with nurses, occupational therapists, and other healthcare professionals to ensure well-coordinated care
- Participate in meetings, conferences, and ongoing education to maintain excellence in patient service
