Job Description

Deliver vital physical therapy care directly to patients in the comfort of their homes by joining a dedicated team focused on patient independence and outcomes. Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

  • Active Physical Therapist license in good standing in the state of practice
  • At least 1 year of recent experience providing Home Health Physical Therapy (field experience highly valued)
  • Proficiency with the Home Care Home Base (HCHB) electronic documentation system preferred
  • Prior OASIS experience required
  • Strong functional assessment and clinical decision-making skills
  • Reliable transportation, valid drivers license, and auto insurance
  • Excellent organizational, time management, and communication abilities
  • Ability to work autonomously and manage a flexible, field-based schedule

Core Responsibilities:

  • Perform comprehensive physical therapy evaluations and OASIS assessments as needed
  • Create and implement personalized treatment plans for mobility, strength, balance, safety, and pain management
  • Administer skilled interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
  • Track patient progress, adjust plans, and update interdisciplinary team and physicians as necessary
  • Educate patients and caregivers on safety, exercises, and home programs
  • Accurately document care through HCHB
  • Coordinate with nurses, occupational therapists, speech-language pathologists, social workers, and aides to ensure high-quality, integrated care
  • Attend case conferences and required agency meetings
