Job Description
Make a direct impact in patients lives by delivering skilled, compassionate physical therapy care in a home health setting. A position is available for a dedicated Physical Therapist to provide field-based services focused on optimizing mobility, independence, and quality of life for homebound individuals within an assigned territory.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Your Experience & Qualifications:
- Current, active Physical Therapist license (in good standing)
- At least 1 year of recent Home Health Physical Therapy experience, preferably with field-based cases
- Familiarity with Home Care Home Base (HCHB) electronic documentation preferred
- Experience with OASIS assessments is required
- Excellent functional assessment and clinical decision-making skills
- Strong communication, organizational, and time management capabilities
- Reliable transportation, valid drivers license, and current auto insurance
- Self-motivated and able to work independently
Role Responsibilities:
- Perform in-depth physical therapy evaluations and OASIS assessments as needed for Start of Care, Resumption, Recertification, and Discharge
- Develop and implement patient-specific treatment plans targeting mobility, strength, balance, and safety
- Deliver skilled therapies such as gait training, fall prevention, transfer training, therapeutic exercise, and neuromuscular re-education
- Reassess patient progress and update plans of care, ensuring collaboration with physicians and interdisciplinary teams
- Educate patients and caregivers on therapeutic exercise, assistive devices, and safety strategies
- Document all care accurately and promptly using HCHB
- Attend case conferences, interdisciplinary meetings, and relevant continuing education
