As a Physical Therapist specializing in home health, you'll play a crucial role in helping individuals regain mobility, strength, and independence while collaborating with a multidisciplinary care team. This contract position is ideal for someone who thrives on autonomy, values patient-centered care, and excels in a dynamic, field-based environment.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Your Impact:

• Provide comprehensive physical therapy evaluations, including OASIS assessments when required.
• Create and implement individualized treatment plans focused on restoring functional mobility, safety, balance, strength, and reducing pain.
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Educate patients and caregivers in exercises, use of assistive devices, home safety, and home exercise programs.
• Monitor patient progress, adjust plans of care as needed, and communicate with physicians and the care team.
• Ensure accurate, timely documentation using Home Care Home Base (HCHB).
• Participate in case conferences, interdisciplinary meetings, and ongoing education.

Your Background:

• Active, unrestricted Physical Therapist license in the state of practice.
• At least one year of recent experience in home health physical therapy (field-based experience preferred).
• Required proficiency in OASIS; experience with HCHB strongly preferred.
• Reliable transportation, valid drivers license, and proof of auto insurance.
• Excellent clinical decision-making and organizational skills; ability to work independently and manage your own field schedule.
