Delivering high-quality care to patients in the comfort of their own homes, this position offers a meaningful opportunity to make a difference as a Home Health Physical Therapist. You'll work with a diverse caseload, providing expert therapy services while building trusting relationships with patients and their families for optimal outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
Desired Qualifications:
- Active Physical Therapist (PT) license in good standing
- Minimum one year of recent home health physical therapy experience
- Proficiency with Home Care Home Base (HCHB) preferred
- Demonstrated OASIS documentation and assessment skills
- Strong functional evaluation and clinical decision-making abilities
- Ability to work independently, traveling to patients within your assigned area (mileage reimbursed)
- Current drivers license, reliable transportation, and valid auto insurance
- Excellent organization, communication, and time management
Key Responsibilities:
- Conduct physical therapy evaluations, including OASIS assessments when appropriate
- Develop and implement individualized care plans to address mobility, strength, balance, gait, and safety
- Administer therapeutic interventions: neuromuscular re-education, exercise, transfer training, gait training, and fall prevention
- Monitor and document patient progress; adjust plans as needed
- Educate patients and caregivers on safe practices, use of assistive devices, and home exercise routines
- Collaborate with nurses, other therapists, social workers, and aides to ensure coordinated, patient-centered care
- Maintain accurate records and compliance with all regulatory requirements
- Participate in interdisciplinary team meetings and ongoing education
