An exceptional opportunity is available for a skilled and compassionate physical therapist to deliver high-quality care in a home health setting. This contract position focuses on supporting patients in their recovery at homeemphasizing functional mobility, strength, balance, and independence. Youll play a crucial role in helping patients achieve their therapy goals while working collaboratively with an interdisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist license in good standing for the state of practice
- At least 1 year of recent experience in home health physical therapy (field experience preferred)
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Strong functional assessment and clinical decision-making skills
- Reliable transportation with valid drivers license and auto insurance
- Comfortable managing an independent, field-based caseload
- Excellent communication, organizational, and time management skills
Primary Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS when applicable
- Develop and implement individualized care plans targeting functional improvements
- Provide skilled interventionstherapeutic exercise, gait and transfer training, pain management, and fall prevention
- Monitor patient progress, adjust plans as needed, and communicate updates to physicians and care team
- Educate patients and caregivers on exercises, safety, and use of assistive devices
- Accurately document all visits and progress in Home Care Home Base
- Collaborate with nurses, occupational therapists, speech-language pathologists, medical social workers, and aides to ensure comprehensive care
- Participate in case conferences and agency meetings
