A rewarding opportunity is available for a skilled Physical Therapist Assistant (PTA) to deliver patient-centered care in a home health setting. Work closely with patients in their homes, supporting their mobility, strength, and independence while collaborating with a licensed Physical Therapist. This contract position offers a structured weekday schedule with the autonomy and variety that comes with field-based work.

Schedule and Details:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (57 patient visits per day)
• Daily travel within an assigned territory (mileage reimbursement provided)
• Utilize electronic documentation via Home Care Home Base (HCHB)

Required Qualifications:

• Active Physical Therapist Assistant (PTA) license in good standing
• At least 1 year recent Home Health PTA experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) preferred
• Familiarity with OASIS documentation processes preferred
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently, demonstrate strong clinical observation, and communicate effectively
• Excellent organization and time management skills

Core Responsibilities:

• Deliver skilled physical therapy interventions according to care plans developed by supervising PT
• Guide patients through therapeutic exercises, gait and balance training, and functional mobility activities
• Monitor, document, and communicate patient progress and any changes in condition
• Educate patients and family members on safety, assistive devices, fall prevention, and ongoing exercises
• Maintain accurate, timely clinical documentation in Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary team and participate in required meetings and case conferences
• Ensure compliance with all policies, regulatory guidelines, and Medicare home health standards

$1,542 - $1,631/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
