Deliver exceptional patient care as a dedicated Home Health Physical Therapist, supporting individuals in their journey toward greater independence in the comfort of their own homes. This contract role offers a rewarding opportunity for compassionate and experienced clinicians to impact patients lives while enjoying autonomy and flexibility in daily practice.

Schedule:

• Full-time, Monday Friday: 8:00 AM 5:00 PM
• 13-week contract term

Role Highlights:

• Conduct comprehensive physical therapy evaluations (including OASIS assessments) and develop individualized treatment plans.
• Manage a caseload averaging 2532 points per week (roughly 57 patient visits daily, adjusted by visit type).
• Deliver skilled interventions such as gait/transfer training, neuromuscular re-education, fall prevention, therapeutic exercise, and pain management.
• Educate patients and caregivers on exercise protocols, safety, use of assistive devices, and home programs.
• Collaborate within a multidisciplinary teamPT, OT, SLP, nursing, medical social work, and aidesto ensure coordinated care.
• Maintain accurate, timely documentation in Home Care Home Base (HCHB) system.
• Attend interdisciplinary meetings and participate in continuing education as required.
• Routinely travel between patient homes within an assigned territory (mileage reimbursement provided).

Qualifications:

• Current Physical Therapist (PT) license in clear standing for the state of practice
• Minimum of one year recent field-based home health PT experience
• OASIS assessment expertise required
• Familiarity with Home Care Home Base (HCHB) electronic documentation preferred
• Strong functional assessment, time management, and communication skills
• Reliable transportation, valid drivers license, and auto insurance

$2,247 - $2,377/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
