An opportunity is available for an experienced and compassionate physical therapist specializing in home health care. This contract position focuses on providing skilled, individualized physical therapy services to adult patients within their homes, promoting greater mobility, strength, and functional independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct in-depth physical therapy evaluations, including OASIS assessments, when appropriate
• Develop and execute individualized care plans tailored to each patient's needs, focusing on mobility, strength, balance, and safety
• Deliver skilled therapy interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Assess patient progress regularly; adjust plans as needed and communicate with physicians and interdisciplinary team members
• Educate patients and caregivers on safe movement, home exercise programs, and proper use of assistive devices
• Maintain compliant, prompt documentation using Home Care Home Base (HCHB)
• Actively participate in interdisciplinary meetings, case conferences, and continuing education
• Adhere to agency protocols, regulatory guidelines, and Medicare home health standards

Required Qualifications:

• Active Physical Therapist (PT) license in the state of practice, in good standing
• At least 1 year of recent experience in home health physical therapy (field experience strongly preferred)
• Proficient with Home Care Home Base (HCHB) and OASIS required
• Outstanding assessment, clinical decision-making, and organizational skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to manage a flexible field-based schedule while working independently

$2,129 - $2,252/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
