Home Health Physical Therapist Assistant (PTA) Contract Opportunity
Location: McDonough, GA

This role offers the opportunity to deliver high-quality physical therapy services in patients homes, helping them improve mobility, strength, and independence under the supervision of a licensed Physical Therapist. The ideal candidate thrives in a field-based environment, managing a caseload independently while providing compassionate, patient-centered care.

Key Responsibilities:

  • Deliver skilled physical therapy treatments in line with prescribed care plans
  • Assist with therapeutic exercises, gait and balance training, transfer techniques, and functional mobility
  • Monitor and document patient progress and treatment responses accurately
  • Communicate changes in patient condition promptly to the supervising Physical Therapist and care team
  • Educate patients and caregivers on safety practices, assistive device use, fall prevention, and home exercise programs
  • Maintain timely and precise documentation using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary teams including nurses, occupational therapists, and social workers
  • Participate in case conferences and team meetings as needed
  • Ensure adherence to agency policies, regulatory standards, and Medicare home health guidelines

Qualifications and Desired Experience:

  • Active Physical Therapist Assistant (PTA) license in good standing for the state of practice
  • Minimum 1 year of recent Home Health PTA experience, preferably field-based
  • Proficiency or familiarity with Home Care Home Base (HCHB) and OASIS documentation is a plus
  • Strong clinical observation, communication, time management, and organizational skills
  • Reliable transportation, valid drivers license, and auto insurance required
  • Self-motivated and able to manage a flexible, field-based schedule independently

Schedule and Contract Details:

  • Full-time, 13-week contract
  • Monday through Friday, 8:00 AM 5:00 PM
  • Caseload of approximately 2532 points per week (57 patient visits daily)
  • Mileage reimbursement included for travel within assigned territory
