Job Description

An opportunity is available for an experienced Home Health Physical Therapist (PT) to provide skilled, patient-centered care in a home-based setting. This full-time, 13-week contract position offers a dynamic caseload where you will help patients enhance their mobility, strength, balance, and overall independence through comprehensive evaluations and tailored treatment plans.

Key Responsibilities:

  • Conduct thorough physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when applicable.
  • Develop and implement individualized plans of care targeting functional mobility, gait training, pain management, transfer training, and fall prevention.
  • Deliver skilled interventions such as therapeutic exercises and neuromuscular re-education.
  • Monitor patient progress, adjust treatments accordingly, and communicate updates to physicians and interdisciplinary team members.
  • Educate patients and caregivers on exercises, safety precautions, assistive device usage, and home exercise programs.
  • Accurately document care in the Home Care Home Base (HCHB) electronic system.
  • Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to ensure coordinated patient care.
  • Participate in case conferences, interdisciplinary meetings, and ongoing education.
  • Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications and Desired Experience:

  • Active Physical Therapist license in good standing for the state of practice.
  • Minimum of 1 year recent home health physical therapy experience; field experience preferred.
  • Experience with OASIS assessments is required.
  • Proficiency in Home Care Home Base (HCHB) is preferred.
  • Strong clinical decision-making and functional assessment skills.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to work independently and manage a flexible, home-based schedule.
  • Excellent communication, time management, and organizational abilities.

Schedule and Location:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Caseload averages 2532 points per week (approximately 57 patient visits daily).
  • Daily travel to patient homes within the assigned territory, with mileage reimbursement provided.
