This contract opportunity calls for a knowledgeable and motivated physical therapist skilled in home health to support individuals on their journey toward optimal independence and function.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Current, active Physical Therapist (PT) license (in good standing)
- At least 1 year of recent experience in home health physical therapy
- OASIS assessment experience required
- Familiarity with Home Care Home Base (HCHB) electronic documentation strongly preferred
- Reliable transportation, valid drivers license, and active auto insurance
- Excellent clinical judgment, strong communication, and organizational skills
- Comfortable working autonomously and adapting to a field-based, flexible role
Core Responsibilities:
- Complete comprehensive physical therapy evaluations in the home setting, utilizing OASIS as appropriate
- Design and oversee individualized care plans aimed at improving functional mobility, strength, balance, and safety
- Administer skilled interventions, including therapeutic exercise, gait and transfer training, fall prevention, and pain management strategies
- Monitor patient outcomes, updating treatment plans as needed and collaborating closely with physicians and the broader care team
- Provide patient and caregiver education on therapeutic techniques, safety guidance, and equipment use
- Accurately document all care provided in Home Care Home Base (HCHB)
- Participate in interdisciplinary meetings, case conferences, and required staff training
