Deliver high-quality care and empower patients to regain independence as an experienced Home Health Physical Therapist. This contract opportunity offers the chance to make a tangible difference in the daily lives of individuals who require skilled rehabilitation within the comfort of their own homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Qualifications:
- Valid Physical Therapist (PT) license in good standing in the state of practice
- At least one year of recent home health physical therapy experience (field experience strongly preferred)
- OASIS assessment experience required
- Experience charting with Home Care Home Base (HCHB) preferred
- Strong clinical skills in functional mobility, assessment, and care planning
- Capable of working independently in a field-based setting
- Reliable transportation, current drivers license, and auto insurance
- Excellent verbal and written communication abilities
Key Responsibilities:
- Conduct thorough PT evaluations, including OASIS assessments for Start of Care, Recertification, and Discharge
- Develop and implement individualized care plans focused on mobility, safety, and overall functional improvement
- Deliver evidence-based interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor patient progress, adjust plans of care as needed, and promptly communicate status updates to physicians and interdisciplinary team members
- Educate patients and caregivers on prescribed home programs, assistive devices, and safety routines
- Maintain clear, timely, and accurate electronic documentation via HCHB
- Attend interdisciplinary team conferences and participate in relevant training opportunities
- Comply with professional standards, agency policies, and Medicare guidelines
