Deliver skilled, compassionate physical therapy services as a Home Health Physical Therapist (PT) supporting patients within their homes. This role focuses on improving patient mobility, strength, balance, and functional independence through individualized care and expert clinical interventions. The ideal candidate is motivated, organized, and adept at managing care autonomously while collaborating with an interdisciplinary team.

Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments as required
- Develop and implement personalized treatment plans emphasizing functional mobility, gait, pain management, and fall prevention
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training
- Monitor patient progress, adjust care plans accordingly, and communicate updates to physicians and care teams
- Educate patients and caregivers on exercises, safety practices, assistive device use, and home exercise programs
- Complete timely, accurate documentation within Home Care Home Base (HCHB) system
- Coordinate care with nurses, occupational therapists, speech-language pathologists, social workers, and aides
- Participate in interdisciplinary meetings and ongoing professional development
- Adhere to agency policies, regulatory standards, and Medicare home health guidelines
Qualifications:
- Active Physical Therapist license in good standing for South Carolina
- Minimum one year of recent home health physical therapy experience, preferably field-based
- Proficiency with OASIS assessments required; Home Care Home Base (HCHB) experience preferred
- Strong clinical decision-making and functional assessment capabilities
- Reliable transportation, valid drivers license, and auto insurance essential
- Excellent communication, time management, and organizational skills
Schedule & Contract Details:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload of approx. 2532 points/week (57 patient visits daily)
- Mileage reimbursement provided for travel within assigned territory
