An opportunity is available for a skilled Home Health Physical Therapist (PT) to deliver personalized, in-home physical therapy services. This role demands a proactive, organized professional who thrives in an independent setting while prioritizing patient-centered care. The therapist will conduct thorough evaluations, design tailored treatment plans, and collaborate with other healthcare professionals to enhance patient mobility, strength, and independence.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments.
• Develop and implement individualized treatment plans targeting mobility, balance, pain management, and safety.
• Deliver skilled interventions such as therapeutic exercises, gait training, neuromuscular re-education, and fall prevention.
• Monitor patient progress, adjust treatment as necessary, and communicate changes with physicians and care teams.
• Educate patients and caregivers on exercises, safety measures, assistive device use, and home exercise regimens.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate within an interdisciplinary team to ensure coordinated patient care.
• Participate in case conferences, team meetings, and mandatory continuing education.
• Adhere strictly to agency policies and regulatory guidelines.

Qualifications and Desired Experience:

• Current Physical Therapist license in good standing.
• Minimum of one year of recent Home Health Physical Therapy experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) is preferred.
• Experience with OASIS assessments is required.
• Strong clinical reasoning and functional assessment skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage scheduling within a field-based role.
• Excellent communication, time management, and organizational abilities.

Schedule and Contract Details:

• Full-time, 13-week contract.
• Monday through Friday, 8:00 AM to 5:00 PM.
• Caseload averages 2532 points weekly (approximately 57 patient visits daily).
• Field-based work involving daily travel to patient homes with mileage reimbursement.
