An experienced Home Health Physical Therapist (PT) is needed to deliver skilled, patient-centered therapy in home settings. This role focuses on improving patient mobility, strength, balance, and functional independence through comprehensive evaluations and individualized treatment plans. Independence, organization, and strong clinical skills are essential as you will manage your caseload while collaborating with interdisciplinary teams.

Key Responsibilities:

• Perform thorough physical therapy evaluations, including OASIS assessments when applicable.
• Develop and implement customized therapy plans targeting mobility, strength, gait, pain, and safety.
• Provide interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
• Monitor patient progress, adjust treatments as needed, and communicate changes to physicians and team members.
• Educate patients and caregivers on exercises, safety measures, assistive device use, and home exercise programs.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate with nurses, occupational and speech therapists, social workers, and aides to coordinate care.
• Participate in case conferences and interdisciplinary meetings.
• Comply with agency policies, Medicare guidelines, and regulatory standards.

Qualifications:

• Active Physical Therapist license in good standing in the practice state.
• At least 1 year of recent home health PT experience; field experience preferred.
• OASIS assessment experience required.
• Proficiency with Home Care Home Base (HCHB) preferred.
• Strong clinical judgment, communication, and organizational skills.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work independently with a flexible field-based schedule.

Schedule & Location:

• Full-time, 13-week contract with Monday to Friday hours, 8:00 AM to 5:00 PM.
• Expect approximately 57 patient visits daily within the assigned territory, with mileage reimbursement provided.
• Work is field-based, providing therapy services during home visits.
