Job Description
Seeking a skilled and compassionate professional to deliver home health physical therapy services. This contract position offers a dynamic field-based role working with a diverse caseload, helping patients achieve greater independence and improved quality of life.
Schedule:
- Full-time, 13-week assignment
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive evaluations, including OASIS assessments as needed
- Develop and execute individualized treatment plans focused on mobility, strength, balance, gait, and pain management
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
- Continuously evaluate patient progress and adjust therapy plans accordingly; communicate changes to physicians and care teams
- Educate patients and caregivers on home exercise programs, safety, and assistive device use
- Maintain accurate, timely documentation using Home Care Home Base (HCHB)
- Collaborate closely with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides for coordinated care delivery
- Attend interdisciplinary meetings and case conferences as required
- Ensure strict adherence to agency policies and Medicare/home health regulations
Qualifications:
- Valid and active Physical Therapist (PT) license in good standing for the state of practice
- At least 1 year of recent home health physical therapy experience (field work preferred)
- Required proficiency with OASIS; experience in Home Care Home Base (HCHB) preferred
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent assessment, documentation, communication, and organizational skills
- Self-motivated with the ability to work independently in a flexible environment
