Make a meaningful difference in patients lives as a skilled Home Health Physical Therapist, delivering therapy in the comfort of patients homes and guiding their recovery journeys toward improved strength, balance, and independence.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS assessments for appropriate milestones such as start of care, resumption, recertification, and discharge.
- Design and implement individualized treatment plans to enhance mobility, build strength, improve safety, and address pain management.
- Deliver interventions including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
- Assess patient progress, update plans of care as needed, and communicate relevant changes to the interdisciplinary team and referring physicians.
- Provide practical education to patients and caregivers on exercise programs, safety techniques, and use of assistive devices.
- Accurately document all assessments and treatments in Home Care Home Base (HCHB).
- Collaborate closely with nurses, OTs, SLPs, social workers, and aides to coordinate care and participate in interdisciplinary meetings and continuing education.
- Adhere to agency policies, federal regulations, and Medicare home health guidelines.
Qualifications:
- Valid and active Physical Therapist license in good standing.
- Minimum one year of recent field-based home health PT experience.
- Proficiency with HCHB documentation strongly preferred.
- Required OASIS assessment experience.
- Strong assessment and clinical decision-making abilities.
- Excellent organization, communication, and independent problem-solving skills.
- Reliable transportation, valid drivers license, and auto insurance.
