Make a meaningful difference by delivering high-quality, patient-centered physical therapy to individuals in their homes as a Home Health Physical Therapist. This contract opportunity requires a dedicated professional comfortable working independently in a field-based environment, collaborating with an interdisciplinary team, and ensuring excellent outcomes for each patient.

Schedule & Contract Details:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Caseload: Approximately 2532 points per week (typically 57 patient visits daily, dependent on visit type)
• Field-based position with daily travel to patient residences (mileage reimbursement included)

Key Responsibilities:

• Conduct comprehensive assessments, including OASIS, to tailor care for start of care, resumption, recertification, and discharge
• Create and implement personalized therapy plans emphasizing mobility, strength, balance, gait, safety, and independence
• Deliver evidence-based interventions: therapeutic exercise, neuromuscular re-education, gait and transfer training, pain management, and fall prevention
• Modify plans as needed, documenting progress in Home Care Home Base (HCHB)
• Educate patients and caregivers on exercise programs, assistive device usage, and safety strategies
• Coordinate care with nurses, OTs, SLPs, social workers, and home health aides
• Ensure compliance with agency guidelines, Medicare, and regulatory standards

Qualifications:

• Active, unencumbered Physical Therapist (PT) license in state of practice
• At least 1 year recent home health PT experiencefield-based experience strongly preferred
• OASIS experience required; proficiency in HCHB preferred
• Excellent assessment, documentation, and time management skills
• Reliable transportation, valid drivers license, and insurance
