Job Description

A skilled Home Health Physical Therapist is needed to provide compassionate, outcome-driven therapy services for individuals seeking to regain independence and improve quality of life. This full-time, 13-week contract offers an engaging, community-focused role with daytime weekday hours.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Core Responsibilities:

  • Complete comprehensive in-home physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as applicable.
  • Develop and implement individualized care plans centered on mobility, strength, safety, gait, pain management, and functional improvement.
  • Utilize skilled interventions including therapeutic exercise, neuromuscular re-education, transfer training, and fall prevention.
  • Regularly assess patient progress, adjust treatment strategies, and communicate updates to overseeing physicians and interdisciplinary team members.
  • Educate patients and caregivers on exercises, safe movement, assistive devices, and home exercise routines.
  • Maintain precise, timely clinical documentation in Home Care Home Base (HCHB).
  • Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides for seamless patient care.
  • Attend case conferences, interdisciplinary meetings, and participate in ongoing education as needed.
  • Adhere to all agency protocols, regulatory standards, and Medicare home health guidelines.

Requirements:

  • Active Physical Therapist license in good standing for the state of practice
  • Minimum of 1 year recent field-based home health PT experience
  • OASIS assessment experience required
  • HCHB system proficiency preferred
  • Strong clinical assessment and decision-making abilities
  • Reliable transportation, valid drivers license, and current auto insurance
  • Exceptional communication, organization, and independent work skills
