Make a meaningful impact in patients lives as a Home Health Physical Therapist by providing individualized care in the comfort of their homes. This is a rewarding full-time, 13-week contract role where your clinical expertise and compassion will empower patients to regain mobility, strength, and independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform thorough physical therapy evaluations and OASIS assessments as required.
• Develop customized treatment plans aimed at improving functional mobility, strength, balance, and safety.
• Deliver skilled, evidence-based interventions such as therapeutic exercises, gait and transfer training, neuromuscular re-education, and fall prevention.
• Regularly assess and adjust care plans to facilitate optimal outcomes, communicating effectively with physicians and interdisciplinary team members.
• Provide patient and caregiver education on exercises, assistive device use, and safety strategies to foster self-management.
• Complete timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate in case conferences, team meetings, and ongoing education sessions.
• Ensure compliance with agency policies and all relevant regulatory standards.

Qualifications:

• Active Physical Therapist (PT) license (in good standing) for the state of practice.
• At least one year of recent home health physical therapy experience (field-based experience preferred).
• OASIS assessment experience required; HCHB proficiency preferred.
• Excellent communication, time management, and clinical assessment abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently and adapt to a flexible field-based schedule.
