Job Description

Deliver transformative care as a Home Health Physical Therapist, using your clinical expertise to enhance patients mobility, independence, and quality of life in the comfort of their own homes. This full-time, 13-week contract role requires compassion, strong organizational skills, and the ability to work autonomously while collaborating closely with an interdisciplinary team.

Schedule:

• Monday through Friday
• 8:00 AM 5:00 PM

Key Qualifications:

• Current, active Physical Therapist (PT) license in good standing
• At least 1 year of recent experience in Home Health Physical Therapy (field-based work strongly preferred)
• Demonstrated proficiency with OASIS assessment tools
• Familiarity with Home Care Home Base (HCHB) electronic documentation system preferred
• Strong skills in clinical decision-making, functional assessment, and care planning
• Reliable transportation, valid drivers license, and proof of auto insurance
• Excellent time management, communication, and organization abilities
• Ability to independently manage a flexible, field-based caseload

Core Responsibilities:

• Travel daily to patients homes to provide skilled physical therapy services (expecting approximately 2532 points per week, 57 visits per day)
• Conduct comprehensive physical therapy evaluations and OASIS assessments as required
• Develop and implement patient-centered care plans to address functional goals for mobility, strength, balance, and safety
• Deliver interventions including therapeutic exercise, gait and transfer training, and fall prevention strategies
• Monitor and document patient progress in HCHB, updating plans as goals are met or needs change
• Educate patients and caregivers in the use of assistive devices and home exercise plans
• Coordinate closely with other healthcare professionals and participate in interdisciplinary meetings and case conferences
