Advance patient care in the home setting by providing skilled physical therapy services to individuals seeking to improve their strength, mobility, and independence. As a Home Health Physical Therapist, youll utilize your clinical expertise to develop individualized treatment strategies, complete comprehensive assessments, and collaborate across disciplines for optimal patient outcomes.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: approximately 2532 points per week (typically 57 patient visits/day)
• Daily travel within assigned territory (mileage reimbursement provided)

Responsibilities:

• Conduct in-home physical therapy evaluations, including OASIS assessments as required
• Create and implement personalized care plans focused on functional mobility, pain management, and safety
• Deliver evidence-based interventions: therapeutic exercise, gait training, neuromuscular re-education, transfer training, and fall prevention strategies
• Regularly assess patients progress, modify treatments as needed, and communicate status updates to physicians and care team
• Educate patients and caregivers on exercises, proper use of assistive devices, and ongoing health maintenance
• Maintain accurate and timely documentation via Home Care Home Base (HCHB)
• Participate in interdisciplinary meetings, case conferences, and required continuing education
• Adhere to all relevant regulatory guidelines and agency policies

Qualifications:

• Active and unrestricted Physical Therapist license in the state of practice
• At least one year of recent home health physical therapy experience; field-based experience strongly preferred
• OASIS assessment experience required; HCHB documentation familiarity preferred
• Strong clinical reasoning, assessment, and interpersonal communication skills
• Reliable transportation, valid drivers license, and appropriate auto insurance
• Capable of working independently and managing schedule in a community-based setting
