Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Your responsibilities will include:

• Performing comprehensive physical therapy evaluations, including OASIS assessments when applicable
• Creating and implementing individualized care plans focused on functional mobility, gait, balance, pain management, and patient safety
• Delivering skilled interventions: therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
• Monitoring and evaluating patient progress, adjusting plans as needed, and communicating effectively with physicians and the interdisciplinary care team
• Educating patients and caregivers on exercises, safety, and use of assistive devices
• Maintaining accurate and timely documentation in Home Care Home Base (HCHB)
• Participating in case conferences and interdisciplinary team meetings
• Ensuring all care meets regulatory standards and agency policies
• Traveling daily between patient homes within assigned territory (mileage reimbursement provided)

Qualifications:

• Active license as a Physical Therapist (PT) in good standing
• At least one year of recent field-based home health physical therapy experience
• Required OASIS assessment and proficiency with functional evaluation
• Familiarity with Home Care Home Base (HCHB) is preferred
• Efficient organizational, communication, and time management abilities
• Independent, self-motivated, and adaptable to a flexible field schedule
• Reliable transportation, valid drivers license, and auto insurance

$2,129 - $2,252/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
