Job Description

Provide compassionate, patient-centered physical therapy care in a home health setting on a 13-week contract located in Leesville, SC. This role requires expertise in assessing, planning, and delivering therapy services that promote patient mobility, strength, and independence in their residences.

Responsibilities:

  • Perform comprehensive initial evaluations to assess physical condition, functional abilities, and limitations.
  • Develop and implement individualized treatment plans in collaboration with physicians and healthcare teams.
  • Administer therapeutic exercises, manual therapy, and modalities tailored to patient needs.
  • Educate patients and caregivers on exercise techniques, transfers, and assistive device usage.
  • Monitor patient progress and adjust treatment approaches as necessary.
  • Maintain accurate and timely electronic medical records following regulatory standards.
  • Collaborate effectively with interdisciplinary team members to coordinate patient care.
  • Comply with ethical, professional, and state and federal regulatory requirements.
  • Travel reliably to patients homes as required.

Desired Experience and Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited program.
  • Current licensure or eligibility for licensure as a Physical Therapist.
  • Prior experience in home health physical therapy.
  • Proficient with HomeCare HomeBase (HCHB) electronic medical record software.
  • Strong clinical judgment, critical thinking, and communication skills.
  • Understanding of Medicare regulations and documentation for home health.
  • Valid drivers license and dependable transportation.
