An opportunity is available for a skilled Home Health Physical Therapist (PT) to provide personalized physical therapy services in patients homes. The role focuses on enhancing mobility, strength, balance, and functional independence through comprehensive evaluations and tailored treatment plans. Position Details:
- Contract: Full-time, 13-week assignment
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points weekly (57 patient visits/day)
- Setting: Patient homes within an assigned territory
- Documentation: Home Care Home Base (HCHB)
- Assessments: OASIS evaluations as required
- Travel: Daily travel with mileage reimbursement provided
Key Responsibilities:
- Perform thorough physical therapy evaluations and OASIS assessments
- Develop and implement individualized treatment plans targeting mobility, strength, balance, gait, and safety
- Deliver skilled interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor patient progress, adjust treatment plans, and communicate changes with the care team and physicians
- Educate patients and caregivers on exercises, safety measures, and assistive device use
- Maintain accurate, timely electronic documentation
- Collaborate with interdisciplinary healthcare professionals to ensure coordinated care
- Adhere to agency policies, Medicare guidelines, and regulatory standards
Qualifications:
- Active Physical Therapist license in good standing within the state
- Minimum 1 year recent Home Health PT experience; fieldwork preferred
- Proficiency with Home Care Home Base (HCHB) preferred
- Experience with OASIS assessments required
- Strong clinical judgment and functional assessment capabilities
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible field schedule
- Excellent communication, time management, and organizational skills
