Step into a rewarding role as a Home Health Physical Therapist, where youll deliver expert rehabilitative care to patients in the comfort of their own homes. Youll be instrumental in improving patients mobility, functional independence, and quality of life, working with diverse cases that require both clinical skill and compassion.

Schedule:

• Full-time contract: 13-week duration
• Monday Friday, 8:00 AM 5:00 PM
• Field-based: travel required to patient homes (mileage reimbursement provided)
• Caseload: Approx. 2532 points per week (typically 57 visits per day)
• Electronic documentation with Home Care Home Base (HCHB)

Key Responsibilities:

• Complete thorough evaluations, including OASIS assessments for new admissions, recertifications, resumptions, and discharges.
• Create and implement tailored therapy plans to address mobility, strength, gait, and balance needs.
• Administer skilled interventions such as therapeutic exercises, neuromuscular re-education, pain management, and fall prevention.
• Educate patients and caregivers on exercise routines, safety strategies, assistive devices, and home programs.
• Continuously assess and document patient progress, adjusting plans as needed and updating care teams and physicians.
• Collaborate with nurses, occupational and speech therapists, medical social workers, and aides to ensure holistic care.
• Maintain accurate, timely records and participate in interdisciplinary meetings and case conferences.

Qualifications:

• Current Physical Therapist license in good standing in the state of practice
• At least one year of recent home health PT experience required
• Proficient with OASIS and, preferably, Home Care Home Base (HCHB)
• Reliable transportation, valid drivers license, and auto insurance
• Ability to self-manage, communicate clearly, and document promptly

$2,727 - $2,885/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
