Make a meaningful impact as a dedicated Home Health Physical Therapist, providing specialized patient care directly in home settings. This contract opportunity empowers you to use your expertise to foster independence and quality of life for individuals needing rehabilitative services.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS assessments as required.
- Develop and implement individualized plans of care to promote mobility, strength, balance, gait, pain control, and safety.
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
- Assess patient progress, adjust care plans as needed, and coordinate with physicians and interdisciplinary teams.
- Educate patients and caregivers on exercise techniques, safety, assistive devices, and home program strategies.
- Maintain accurate, timely electronic documentation via Home Care Home Base (HCHB).
- Collaborate with nurses, occupational and speech therapists, social workers, and aides to coordinate holistic care.
- Attend case conferences and engage in required interdisciplinary meetings and continuing education.
- Comply with agency policies, regulatory standards, and Medicare guidelines for home health care.
Qualifications:
- Active Physical Therapist (PT) license in good standing for the state.
- Minimum of one year of recent home health physical therapy experience.
- Proficiency with Home Care Home Base (HCHB) and OASIS required.
- Demonstrated clinical assessment and decision-making skills.
- Independent, organized, and adaptable with strong communication and time management abilities.
- Reliable transportation, current drivers license, and auto insurance required.
