An exciting opportunity is available for an experienced and compassionate Home Health Physical Therapist to provide patient-centered care in a dynamic field-based setting. As a Physical Therapist, youll help patients regain mobility, strength, and independence within the comfort of their own homes, working alongside an interdisciplinary team to achieve optimal outcomes.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes within an assigned area (mileage reimbursement provided)
• Caseload: Approximately 2532 points per week (typically 57 visits per day, depending on visit types)

Qualifications:

• Active Physical Therapist license in good standing
• Minimum of 1 year of recent home health physical therapy experience (field experience preferred)
• Experience with OASIS assessments required
• Familiarity with Home Care Home Base (HCHB) electronic documentation system preferred
• Strong clinical assessment and decision-making skills
• Exceptional communication, time management, and organizational abilities
• Reliable transportation, current drivers license, and auto insurance
• Ability to work independently and manage a flexible, mobile schedule

Responsibilities:

• Complete comprehensive physical therapy evaluations, including OASIS assessments where applicable
• Develop and implement individualized treatment plans focused on functional mobility, balance, safety, and pain management
• Deliver skilled interventions (therapeutic exercise, gait training, neuromuscular re-education, transfer training, fall prevention)
• Educate patients and caregivers on exercise programs, safety, and assistive devices
• Maintain timely and accurate documentation using HCHB
• Collaborate with other healthcare professionals to ensure coordinated care
• Participate in interdisciplinary meetings and continuing education as required
• Comply with all agency policies and home health regulatory standards

$1,609 - $1,702/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
