Job Description
An experienced and compassionate Home Health Physical Therapist is needed for a 13-week contract, providing skilled care to patients in their homes. This role is ideal for a self-motivated clinician who thrives on independence and enjoys supporting patient recovery while maintaining a high standard of care.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based position requiring daily travel to patient homes (mileage reimbursement provided)
- Caseload: Approx. 2532 points/week (typically 57 patient visits per day, type-dependent)
Desired Experience & Qualifications:
- Active Physical Therapist license (PT) in good standing for the state of practice
- At least 1 year of recent, relevant home health physical therapy experience
- Proficiency with Home Care Home Base (HCHB) preferred
- Required OASIS assessment experience
- Proven clinical decision-making and functional assessment skills
- Reliable transportation, current drivers license, and valid auto insurance
- Ability to work both independently and as part of an interdisciplinary team
- Excellent communication, time management, and organizational skills
Key Responsibilities:
- Conduct comprehensive patient assessments, including OASIS evaluations, and develop individualized plans of care
- Deliver skilled physical therapy interventions to promote mobility, strength, balance, and safety
- Monitor and document patient progress using HCHB, updating treatment as needed
- Educate patients and caregivers regarding exercises, safety, use of assistive devices, and home programs
- Coordinate with nurses, therapists, and home health aides to support optimal patient outcomes
- Participate in interdisciplinary meetings and maintain compliance with all regulatory and agency guidelines
