A rewarding opportunity awaits an experienced and compassionate Physical Therapist specializing in home health care. In this contract-based role, you will provide skilled therapy services directly to patients in their homes, supporting their journey toward improved mobility and greater independence. Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract assignment
• Field-based position with daily local travel (mileage reimbursement provided)

Key Responsibilities:

• Perform comprehensive patient evaluations, including required OASIS assessments for start of care, recertification, and discharge.
• Develop and implement individualized care plans focusing on functional mobility, strength, gait, balance, pain management, and safety.
• Deliver hands-on interventions, including therapeutic exercise, gait and transfer training, fall prevention, and neuromuscular re-education.
• Monitor patient progress, update treatment plans as appropriate, and communicate changes to physicians and interdisciplinary team members.
• Educate patients and caregivers on home exercise programs, use of assistive devices, and fall prevention.
• Complete timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate with registered nurses, occupational therapists, speech clinicians, social workers, and home health aides to coordinate care.
• Attend case conferences and interdisciplinary meetings as required, staying current with best practices and home health regulations.

Preferred Qualifications:

• Active state licensure as a Physical Therapist (PT) in good standing.
• At least one year of recent home health physical therapy experience preferred.
• Experience with Home Care Home Base (HCHB) and OASIS required.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and efficiently in a field-based setting.

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
