Job Description

Provide skilled physical therapy care as a Home Health Physical Therapist (PT), delivering patient-centered treatment in residential settings. This full-time, 13-week contract position requires independent management of a field-based caseload, focusing on improving patient mobility, strength, balance, and functional independence.

Location:
This role is based in Lake City, SC, a community known for its welcoming atmosphere and access to natural scenic beauty. The area offers a blend of small-town charm with nearby urban amenities, providing a balanced lifestyle for healthcare professionals.

Schedule:

  • Monday through Friday
  • 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform comprehensive physical therapy evaluations including OASIS assessments.
  • Develop and implement individualized plans of care targeting mobility, gait, strength, pain management, and safety.
  • Provide skilled interventions: therapeutic exercise, neuromuscular re-education, gait/transfer training, and fall prevention.
  • Monitor patient progress and adjust treatment plans as needed, communicating changes to physicians and care teams.
  • Educate patients and caregivers on exercises, safety protocols, and use of assistive devices.
  • Maintain accurate documentation using Home Care Home Base (HCHB).
  • Collaborate effectively with a multidisciplinary team to ensure coordinated care.
  • Adhere to agency policies, regulatory standards, and Medicare guidelines.

Qualifications and Desired Experience:

  • Active Physical Therapist license in good standing for the state.
  • Minimum 1 year of recent home health PT experience; field experience preferred.
  • OASIS assessment experience required.
  • Proficiency with Home Care Home Base (HCHB) preferred.
  • Strong clinical judgment and functional assessment skills.
  • Reliable transportation, valid driver's license, and auto insurance.
  • Ability to work independently with excellent organizational and communication skills.
