An opportunity exists for an experienced Home Health Physical Therapist (PT) to provide skilled, patient-centered physical therapy services in patients' homes. This full-time contract role involves evaluating and treating patients to improve mobility, strength, balance, and overall functional independence. The ideal candidate is self-motivated, organized, and comfortable working independently within an interdisciplinary healthcare team.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments as needed.
• Develop and implement individualized plans of care targeting functional mobility, pain management, gait training, and safety.
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, transfer training, and fall prevention.
• Monitor patient progress, adjust treatments, and communicate status updates to physicians and care team members.
• Educate patients and caregivers on exercises, assistive device use, and safety strategies.
• Document patient care accurately and timely using Home Care Home Base (HCHB).
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and home health aides.
• Participate in case conferences, team meetings, and continuing education.
• Follow agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Active Physical Therapist license in good standing in the state.
• Minimum 1 year recent home health physical therapy experience; field experience preferred.
• Proficiency with OASIS and familiarity with Home Care Home Base (HCHB) preferred.
• Strong clinical decision-making and functional assessment abilities.
• Valid drivers license, reliable transportation, and insurance for patient home visits.
• Excellent communication, organizational, and time management skills.

Schedule & Contract Details:

• Full-time, 13-week contract.
• Monday to Friday, 8:00 AM 5:00 PM.
• Caseload approximately 2532 points weekly (57 visits daily).
• Field-based with daily travel to patient homes; mileage reimbursement provided.
