An opportunity exists for an experienced Home Health Physical Therapist (PT) to provide skilled, patient-centered physical therapy services in patients' homes. This full-time contract role involves evaluating and treating patients to improve mobility, strength, balance, and overall functional independence. The ideal candidate is self-motivated, organized, and comfortable working independently within an interdisciplinary healthcare team.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments as needed.
- Develop and implement individualized plans of care targeting functional mobility, pain management, gait training, and safety.
- Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, transfer training, and fall prevention.
- Monitor patient progress, adjust treatments, and communicate status updates to physicians and care team members.
- Educate patients and caregivers on exercises, assistive device use, and safety strategies.
- Document patient care accurately and timely using Home Care Home Base (HCHB).
- Collaborate with nurses, occupational therapists, speech therapists, social workers, and home health aides.
- Participate in case conferences, team meetings, and continuing education.
- Follow agency policies, regulatory standards, and Medicare home health guidelines.
Qualifications:
- Active Physical Therapist license in good standing in the state.
- Minimum 1 year recent home health physical therapy experience; field experience preferred.
- Proficiency with OASIS and familiarity with Home Care Home Base (HCHB) preferred.
- Strong clinical decision-making and functional assessment abilities.
- Valid drivers license, reliable transportation, and insurance for patient home visits.
- Excellent communication, organizational, and time management skills.
Schedule & Contract Details:
- Full-time, 13-week contract.
- Monday to Friday, 8:00 AM 5:00 PM.
- Caseload approximately 2532 points weekly (57 visits daily).
- Field-based with daily travel to patient homes; mileage reimbursement provided.
