Bring your expertise and passion for patient-centered care to a dynamic, field-based role as a Home Health Physical Therapist. This full-time, 13-week contract offers an opportunity to make a direct impact by delivering skilled therapy services in the comfort of patients homes.
Schedule & Details:
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (about 57 patient visits per day)
- Home-based visits within assigned area, mileage reimbursed
- Utilize Home Care Home Base (HCHB) for documentation and OASIS for key assessments
What You'll Do:
- Complete thorough physical therapy evaluations, including OASIS assessments as needed
- Design and implement individualized plans of care targeting improved functional mobility, strength, balance, gait, and pain management
- Administer interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies
- Track progress, modify treatment as necessary, and communicate updates to physicians and interdisciplinary care teams
- Provide hands-on education for patients and family members regarding safety, exercise, and use of assistive devices
- Maintain detailed, timely clinical documentation within HCHB
- Participate in case conferences and continued education opportunities as required
- Consistently adhere to regulatory and Medicare guidelines
What We're Looking For:
- Active Physical Therapist (PT) license (in good standing)
- At least 1 year of recent home health physical therapy experience (field experience preferred)
- Hands-on OASIS assessment experience required; HCHB documentation proficiency preferred
- Strong assessment, clinical decision-making, and organization skills
- Dependable transportation, current drivers license, and auto insurance
