Deliver high-quality patient care in a rewarding and autonomous role as a Home Health Physical Therapist. This contract opportunity allows you to make a tangible impact on the lives of individuals needing physical therapy services in the comfort of their homes. You'll utilize your clinical skills to support mobility, functional independence, and patient safety. Working closely with interdisciplinary team members, you will guide patients and caregivers on the journey toward optimal health.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Essential Qualifications:

• Active Physical Therapist (PT) license in good standing for the state of practice
• At least 1 year of recent experience in home health physical therapy, with direct field experience preferred
• OASIS assessment expertise
• Familiarity with Home Care Home Base (HCHB) documentation system strongly preferred
• Strong assessment, clinical decision-making, and functional evaluation capabilities
• Independent, organized, and self-driven with excellent communication and time management skills
• Reliable transportation, valid drivers license, and current auto insurance required

Core Responsibilities:

• Complete comprehensive patient evaluations, including OASIS assessments as needed
• Develop and implement tailored therapy care plans focused on functional rehabilitation, mobility, and safety
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait, and transfer training
• Monitor patient progress, adjusting care plans and collaborating with care teams across disciplines
• Educate patients and family members regarding exercises, fall prevention, safety techniques, and equipment usage
• Maintain timely and accurate documentation using HCHB
• Attend interdisciplinary meetings, case conferences, and participate in ongoing professional development
• Ensure adherence to agency policies, regulatory requirements, and Medicare home health guidelines
